In case of intercollegiate athletic injury information

In the event of a college related intercollegiate athletic injury that would force a student athlete to seek medical attention, the procedure is as follows.

1. Student Athlete must give the PRIMARY (your insurance co.) insurance information to the the medical provider (doctor, hospital, imaging, etc.) so they can file with your insurance company.

2. Student Athlete sees athletic trainer to give a detailed report as to what happened and sign a claim form.

3. Give athletic trainer a copy of the first itemized bills from any and all providers (hospitals, doctors, imaging, etc.).

4. When YOUR PRIMARY INSURANCE has paid their portion bring the athletic trainer a copy of the explanation of payment showing what is owed, how much they paid, and what is still owed to that provider.

We can NOT use only the monthly statement showing the balance due to send to our school athlete insurance company.

In order for your bills to get paid this has to be done in a timely manner.

Please have athlete bring insurance card along so a copy can be made. Card will be returned to athlete.

Coverage will be verified. If coverage cannot be verified student will not be allowed to practice or participate in sport.

If your present coverage is through an HMO provider, please have them verify there will be coverage for your child while they are here attending Iowa Central Community College and participating in athletics.
